Form for Pro Hac Vice Admission
Use this online form if you are an attorney who is qualified to practice law in
another state or in a foreign jurisdiction, is not admitted to practice law in Pennsylvania,
and is seeking to be specially admitted to the Bar of the Commonwealth of Pennsylvania
in order to appear before a Pennsylvania court in connection with a particular case.
The admission fee is $200.00 per attorney per case.
INSTRUCTIONS
Before you start, please gather the following information:
- The applicant attorney’s license number in the primary jurisdiction where they are
licensed to practice law. If the license number is unknown, contact the attorney
registrar in the primary jurisdiction where the attorney is licensed.
- The license number of the Pennsylvania attorney of record (sponsoring attorney).
You may obtain the license number from the Pennsylvania Disciplinary Board’s website:
www.padisciplinaryboard.org.
- The number, name and court of record of the Pennsylvania case on which the applicant
attorney wishes to enter their appearance.
- If the client being represented has been granted in forma pauperis status, evidence
of this status must be attached to the application. Acceptable documentation includes
a copy of the motion requesting or order granting leave to proceed in forma pauperis.
